Developmental Connections contracts with the Missouri Department
of Mental Health, Division of Developmental Disabilities to provide
case management services for area persons with developmental
disabilities. Case Management, also known as Service Coordination, helps locate and integrate services with community
agencies and authorizes individually planned supports and services. These services and programs are all
designed toward enabling persons to progress toward normal living and to develop their capacity, performance
and relationships with others. Effective January 1, 2011, Developmental Connects, through its Service Coordination program will be the provider of all case management services for persons with developmental disabilities in Taney, Ozark and Stone Counties.
What is Case Management?
Case Managers assist individuals who have been determined eligible for services from the Missouri Division of
Developmental Disabilities in accessing the services they need.
Case Managers are professionals who are trained in the field of mental health or a closely related field. Each
individual who applies for services from the Division of Developmental Disabilities is assigned a Case Manager
who will help coordinate, link and connect eligible individuals to resources and services as identified in the
person-centered planning process.
